1.   How do I

Apply for graduation?
Apply in person at the Registrar’s office, through WebAdvisor, or go to the Registrar’s website, fill out the “Graduation Application” form, print, sign and submit to the Registrar’s Office by fax, mail or hand carry.  There is no fee for graduation.  Deadlines to apply for graduation can be found on the Registrar’s website calendar.

Order a duplicate diploma?
Order in person at the Registrar’s office or go to the Registrar’s website, fill out the “Duplicate Diploma” form, print, sign and submit to the Registrar’s Office; fax if paying with credit card, mail or hand carry in your order form. All orders must be paid in advance, before an order can be placed.  The cost is $50.00 per diploma, and takes 6-8 weeks for processing. 

Change my major/minor for my undergraduate degree?
“Major/Minor” forms must be submitted to the Registrar’s office with your advisor’s signature.  Changing Majors requires the Department Chair’s signature of the major to which you are applying; fax, mail or hand carry the signed form.

Order official OLLU transcripts?
There are two ways to order official transcripts 1) come by the Registrar’s Office and complete a transcript request form 2) go to the Registrar’s website, click on “Transcript” form, fill out, print, sign and submit to the Registrar’s Office by fax, mail or hand carry.

Change my name, address or phone number?
Obtain the “Name Change” form from the Registrar’s website, fill out, print, sign and submit with required documentation to the Registrar’s Office by fax, mail or hand carry. Address/phone? Obtain the “Address Change” form, fill out, print, sign and submit to the Registrar’s Office by fax, mail or hand carry.

Order an Enrollment Verification?
Obtain the “Enrollment Verification” form from Registrar’s website, fill out, print, sign and submit to the Registrar’s Office by fax, mail or hand carry.

Get a grade report (Report Card)?
You may obtain your grades through WebAdvisor.
  If you need an official grade report from the Registrar’s Office, obtain the “Request for Grade Report” form from Registrar’s website, fill out, print, sign and submit to the Registrar’s Office by fax, mail or hand carry.

 

Change advisors or find my advisor?
You would have to talk to your Department before requesting a change in Advisor then the department will inform the Registrar’s Office of a change if necessary. The advisors name will be printed on the student’s degree audit. The student can also contact their department or the department chair for the name of their advisor or they can contact the Registrar’s office.

 

Change my concentration for my Master’s degree?
Obtain the “Change of Concentration” form from the Registrar’s website, fill out, print, obtain all signatures and submit to the Registrar’s Office by fax, mail or hand carry.   How do I change to a different masters program?  You must apply to the program with the Admissions Office or submit your application online.

Register for classes?
If you are a current student you may register online through WebAdvisor or in person at the Registrar’s Office.  All undergraduate students must visit with their advisor in order to obtain access to register online and select course schedule. Undergraduate students are required to obtain their advisor’s signature on approval form if registering in person.  If you are a new student, contact your recruiter.

Faculty may at their discretion add pluses or minuses to letter grades, these distinctions are not calculated in the grade point average.

2. What is FERPA?

FERPA
The Family Educational Rights and Privacy Act (FERPA) is a Federal Law (also known as the Buckley Amendment) which protects the privacy of a student’s educational records.  FERPA applies to all educational agencies or institutions that receive funds under any program administered by the Secretary of Education.  FERPA entitles students to withhold release of directory information.  Directory information is defined as “information contained in an education record of a student which would not generally be considered harmful or an invasion of privacy if disclosed.” (1988 Final Regulations).  Our Lady of the Lake University public or directory information includes:  student’s full name, address (local, permanent, e-mail), telephone listings (local, permanent), email address, date and place of birth, major field of study, degrees and awards received, dates of attendance, most recent school attended, classification, photographs, weight and height of members of athletic teams and participation in officially recognized activities and sports.  Please see the Registrar’s website under FERPA for additional information.

How can parents have access to their daughter/son’s information?
Students should request in writing that their parent/guardian have access to specific information.  Students may obtain the “Student Consent to Release Educational Records” form from the Registrar’s website, fill out, print, sign, observe specific instructions as to who can or cannot view their directory information and submit to the Registrar’s Office by fax, mail or hand carry.  Dependent status is verified with Financial Aid office.

How do I put a directory hold? 
Obtain the “Request to prevent disclosure of Directory Information” form from the Registrar’s website, fill out, print, sign and submit to the Registrar’s Office by fax, mail or hand carry.

3. What is a

Letter of Completion or a Letter of Anticipated Completion?
A Letter of Completion will state what Degree has been earned and the date the degree was conferred. A Letter of Anticipated Completion will say what degree the student is working towards and the anticipated date of completion for that degree. To receive either letter the student must obtain the “Anticipated Letter of Completion/Letter of Completion” form from the Registrar’s website, fill out, print, sign and submit it to the Registrar’s office by fax, mail or hand carry.

Letter of Standing Request?
This form should be completed prior to taking a course at another institution. It allows Registrar’s Office personnel to review & determine if the course is transferable & meets the degree requirement for the student. Obtain the “Request to Take Courses at another College/University” form at the Registrar’s website, fill out, print, obtain signatures and submit to the Registrar’s Office for approval by fax, mail or hand carry.

Degree Audit?
A degree audit is a report that informs a student what courses are needed to complete their degree requirements.  You may obtain a degree audit through WebAdvisor or in the Registrar’s Office.
