PREPARE FOR YOUR VISIT
Only current student, faculty, or staff can see the Nurse Practitioner. You will be required to show your current University ID. Patients are required to have an updated and complete medical record before seeing the Nurse Practitioner. Printable forms are available on this page which allow for less wait time and improved assistance from the Office of Health Services. These forms are available in PDF format and require Adobe software to view.

Notice of Privacy Practices
Patients visiting the office to obtain medical records (either from OLLU or another medical facility) must fill out a Release of Medical Information
Patients must present a valid driver's license in order to pick up their medical records. Request for medical records should be done at least a few days in advance.
The communication of medical information via the Internet and/or e-mail is prohibited by the Office of Health Services. The above forms are provided for download, print, and hand (US Postal) delivery only.
