Meningitis Info
HEALTH INSURANCE WAIVER
Waiver form can be completed online by logging in to MyOLLU’s portal and completing waiver form under “Links to Forms”.
After the waiver form is completed and submitted, an email will be sent to the email on file, confirming that you have completed and submitted your waiver. Per Our Lady of the Lake University policy, International Students are not allowed to waive coverage.
Late waivers will not be considered and posted charge will remain. Check your email for due dates.
Meningitis
ALL (undergraduate, graduate, Ph. D, and weekend college) students currently enrolled or enrolling in OLLU must submit evidence of vaccination against bacterial meningitis or the appropriate exemption form to Health Services.
All Students are required to turn in the Meningitis Vaccination Verification Form. This verification form is required from every student at OLLU, regardless of age.

You can obtain the meningitis vaccine at Health Services, your primary care physician, Walgreens, H-E-B's, and other convenient care clinics (i.e. Concentra, Metropolitan Health District, etc.).
